ADA Related Forms

The Americans with Disabilities Act (ADA) and Section 504 are federal anti-discrimination statutes that provide civil rights protection for persons with disabilities. Among other things, the legislation requires that all students with disabilities be guaranteed a learning environment that provides for reasonable accommodation of their disabilities.

If you believe you have a disability requiring an accommodation, you can submit a request for institutional accommodations here. If you have any questions, please contact the Disability Support Services (DSS) office at ADA@redlandscc.edu or call 405.422.1266. In order to receive academic accommodations for a disability, the student must provide appropriate documentation of their disability to the DSS office. This information may be mailed, emailed, or hand-delivered.

If you are requesting an emotional support animal to live with you in campus housing, please contact the Disability Support Services (DSS) office at ADA@redlandscc.edu or call 405.422.1266 for further information.

To file a grievance, please complete the Student Accommodation Grievance Form.

A variety of technologies are used for course delivery and for assessment of learning, particularly in online courses and blended courses (partially online). If you need assistance in accessing online materials, contact your course instructor or the DSS office at 405.422.1235.

 

  1. Submit request for institutional accommodations here.
  2. Submit *required documentation to Disability Support Services (DSS), located in Admissions and Advising, in the Ray Porter building, office SS-005.
  3. Student to be notified of decision for accommodation(s) by the second week of the semester.
  4. Finalize accommodations pending approval.
  5. Student is required to submit accommodations form each semester.

*Documentation must provide information on specific functional limitations, and demonstrate the need for specific accommodations. Diagnosis by a physician or other licensed medical specialist is required. A student's Individualized Education Plan (IEP) will be considered as documentation when it identifies the disability along with the functional limitations. 

 

Faculty has the right to:

  • Refuse requests for accommodations that have not been processed through the Disability Support Services (DSS) office.
  • Consult with DSS if they believe an accommodation request fundamentally impacts the integrity of the course/program; creates undue financial burden on the school, or if it causes a safety issue.
  • Obtain appropriate consultation with DSS about how to meet the needs of a student with a disability.
  • Receive official, and as possible, advance notice of the need for accommodations.

Faculty has the responsibility to:

  • Foster an accepting classroom environment for students with disabilities characterized by non-discrimination.
  • Cooperate with DSS when a request for accommodation is made. Discuss needs individually and on a timely basis with students requesting accommodations.
  • Provide reasonable accommodations in a timely manner to students who have been approved for services by DSS.
  • Ensure instructional materials, methods and programs are accessible, effective and equally integrated. Faculty may consult with DSS to provide accessible materials.
  • Treat and protect all disability-related information as confidential information.

Please note: Faculty do NOT have the right to ask students if they have a disability, nor about the nature of the disability relevant to their granted accommodations. However, if students choose to disclose their disability, this information should be treated confidentially.

Redlands Community College through Disability Support Services has the right and responsibility to:

  • Maintain the College’s academic standards.
  • Discuss student’s eligibility given signed consent.
  • Select from among equally effective/appropriate accommodations, adjustments, and/or auxiliary aids in consultation with the student.
  • Deny requests for accommodations, academic adjustments, and/or auxiliary aids that are inappropriate or determined unreasonable based on if the accommodation:
  • May pose a direct threat to the health and safety of others.
  • Constitutes a substantial change or alteration of an essential element of a course/program standard.
  • May pose an undue financial or administrative burden on the College.

 

Every student registered with Disability Support Services (DSS) has the right to:

  • Be free from discrimination on the grounds of having a disability and free from retaliatory discrimination.
  • Be assured that information about a disability is held confidential. It will not be shared with anyone unless permitted by the student in writing, except as required by law.
  • Have equal opportunity and access to educational programs, services, activities, and facilities available at all Redlands Community College campuses.
  • Have reasonable and appropriate accommodations, academic adjustments, and/or auxiliary aids as determined on a case-by-case basis, where the student’s disability limits access, participation, or ability to benefit.
  • Meet privately with faculty to discuss needed accommodations and address any other concerns.
  • Be treated equally with other students regarding grades and class participation.
  • Appeal decisions regarding accommodations and auxiliary aids.

Every student registered with DSS has the responsibility to:

  • Identify self in a timely manner to DSS as a student with a disability requesting academic accommodations.
  • Submit request for institutional accommodation(s). Form is available here. All supporting documentation must be on file for requests to be evaluated.
  • Discuss accommodations with instructor(s) and participate in determining how the accommodations will be implemented.
  • Notify DSS immediately when accommodations are not provided.
  • Make arrangements with DSS staff for assistance with equipment, tutoring and testing.
  • Meet the same academic and student conduct standards as other students, and ask for assistance and accommodations in an appropriate manner.

Absences due to medical conditions relating to pregnancy will be excused for as long as deemed medically necessary and students will be given the opportunity to make up missed work. A student is allowed to return to the same academic and extracurricular status as before the medical leave began. Mothers returning to work following the birth of a child will be provided reasonable break periods to express breast milk. REDLANDS is dedicated to providing a Lactation room for any student, employee or visitor to use to express milk or to be used to feed a child if desired. Title IX

Redlands Community College Title IX Mission Statement 
The mission of the Title IX Office is to fulfill its commitment to protect all persons who visit, participate, work at Redlands Community College programs and activities from discrimination or harassment based upon their gender as outlined in Title IX of the Educational Amendment of 1972 to the 1964 Civil Rights Act. Redlands Community College prohibits any type of gender equity discrimination.   Redlands takes acts of dating violence, domestic violence, harassment, threats and bullying on the basis of sex, retaliation, sexual harassment, sexual misconduct, and stalking extremely seriously and strongly encourages individuals to report these acts to the necessary authorities.

The Title IX Coordinator has been designated to handle inquiries regarding sex discrimination, including sexual harassment, sexual assault, pregnancy, domestic violence, and stalking.

 

Title IX Policy

“No person in the United States shall, on the basis of sex, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving Federal financial assistance.”

Title IX Report

Sexual Assault Anonymous Reporting Form

Using the Sexual Assault Anonymous Reporting form, victims/survivors of dating violence, domestic violence, retaliation, sexual harassment, sexual misconduct, and/or stalking may file an anonymous report with the Office of Title IX, whether or not the victim of a sexual assault decides to file disciplinary and/or legal charges against the offender at a later date.

This form is also used to report alleged discrimination against pregnant and parenting individuals as well as transgender individuals.

Please select the appropriate answer to each question. Then, follow the directions at the end of the form.

To keep information anonymous, do NOT include names of victim or perpetrators. If names are included, this is no longer considered an anonymous report and an investigation may be conducted by the Redlands Community College Title IX Coordinator.

A report will prompt the Title IX Coordinator to notify the complainant about supportive measures and to discuss the process to file a formal complaint, but does not prompt a Title IX investigation.

Formal Complaint

A formal complaint is when a complainant files a signed document alleging sexual harassment and requests the college to investigate the allegation.

There is not a statute of limitations on filing complaints within the Office of Title IX. If you later decide to file disciplinary and/or legal charges, you may initiate this process by contacting the Title IX Coordinator.

At Redlands, we believe in fostering a culture of diversity, equity, and inclusion that welcomes and respects everyone for who they are and who they will become. We have established a process that allows students to update a legal name or select a preferred name. This option has been developed to respond to the needs of those who prefer to use their middle name instead of their first name, people who use nicknames of a legal name, international students that might use an anglicized name, or students who use a name that affirms their gender identity.

Request Legal Name Change

Redlands understands that you may choose to legally change your name for various reasons including marriage, divorce, court order or adoption.

Collect Your Required Documentation

For legal name changes, Redlands requires different documentation depending on the situation. Please call 405.422.6238 if you have questions.

  • Redlands Student ID or Government Issued Photo ID (passport, driver's license, military ID)
  • Social Security Card
  • Driver’s license reflecting new name, divorce decree, or marriage certificate
  • Supporting court documents
  • Other documents as requested

For security purposes, you MUST email from your assigned Redlands email address. Redlands will not process requests from personal email addresses.

 

What is a preferred name?
A first name, other than the legal first name, by which the student or employee prefers to be identified, which has been designated by the student or employee.

Why are preferred names important?
Calling an individual by their preferred name shows respect. Use of preferred names contribute to providing an empowering, safe, and inclusive environment. Names are an important part of an individual’s identity. Therefore, using the wrong name can cause a person to recall a time or experience when they couldn’t openly express who they are. Additionally, many names are gendered. Use of a preferred name often better aligns with individual gender expression and prevents that person from being subjected to misgendering.

Is a preferred name required?
No. If an individual regularly uses their legal name, there is no reason to request a preferred name.

Can any name be requested as a preferred name?
Yes. You may choose the name of your personal preference. However, Redlands retains the authority to deny inappropriate preferred names including, but not limited to, those that contain offensive or derogatory language, inappropriate use of special characters, or is being used for misrepresentation, etc. Preferred names may also not be used for commercial purposes and therefore may not include a company name, group name, catchphrase, or message.

Will the preferred name be used everywhere at Redlands?
No. Students’ preferred name will be used whenever possible in communications and reporting, except where the use of the legal name is necessitated by Redlands business.

For legal reasons, there are three categories in which preferred names cannot be used:

  • Official transcripts
  • Documents related to Financial Aid
  • Documents related to International Student Services

In what systems will my preferred name appear once it is updated?
Your preferred name will appear in these systems once updated:

  • Self Service
  • Canvas
  • Redlands Webmail
  • Class Rosters
  • Diplomas

Can I update my Student ID card to include my preferred name?
Yes. If you have completed the Preferred Name Change process to designate your preferred name through the Registrar, please visit the admissions and advising office on any campus and request to use your preferred name on your Redlands ID.

Individuals who are issued ID cards with a preferred name that is different from their legal name should be cautioned that such cards are issued for the purpose of Redlands business only, and that identification cards issued by Redlands Community College are not considered government-issued identification.

In accordance with 34 CFR Part 106.45(1)(iii) & 106.45 (b)(10)(1)(D), Redlands Community College is required to publicly post all training materials used by those addressing Title IX complaints. Employees should contact the Title IX Coordinator, Title.IX@redlandscc.edu, to log the training that they have completed.
